Becoming a Houston IVF Patient

The first step in overcoming infertility is to schedule an appointment with one of our Houston fertility specialists who are experts in the field of reproductive medicine. The first appointment will be an important one and both partners are encouraged to attend.

If you would like to make an appointment please call our Houston fertility center at (713) 465-1211 to be scheduled immediately or you may complete the following form and e-mail it to our office.

Please read the Welcome document on the Documents tab. Complete the Patient Intake Form and fax it to us at (281) 822-2681 or mail it to us at the address shown below. We strongly recommend that you do not send any confidential patient information to us by e-mail.

Your physician will discuss Genetic Testing during your initial consultation.  Please review the Carrier Screening Information so you may ask any questions during your visit. You will be asked to sign the Informed Consent/Refusal for Genetic Carrier Screening at your appointment.
