Becoming a Houston IVF Patient
The first step in overcoming infertility is to schedule an appointment with one
of our physicians who are experts in the field of reproductive medicine.
The first appointment will be an important one and both partners are encouraged
to attend.
The initial consultation takes about an hour and costs $200. For our
out-of-state patients, we offer an initial consultation via phone for $210.

scheduled immediately or you may complete the following form and e-mail it to
our office.
Welcome to Houston IVF! We will contact you via phone and/or e-mail to schedule your new
patient appointment and provide you with information you need prior to your
first appointment.
You will be contacted within a week to set up your initial consultation. If you call us for your initial appointment, your consultation will be set up immediately. Please download the forms listed below, fill them out and fax them to 713-550-1475 prior to your scheduled appointment (Attention: Jean). * Note: Please make sure your New Patient Forms are received at our office 48 hours prior to your appointment.
